Home health care is a service provided on an intermittent basis that allows homebound patients under the care of a physician or certified nurse practitioner, to remain in the comfort of familiar surroundings.
Mid-Delta is dedicated to improving healthcare in the home. Through the use of advanced technology such as Resideo® Tele-health Monitoring and continuous training of our nurses and staff, we have been able to significantly decrease emergency room visits and hospital readmission rates. We have a proven track record of over 40 years of experience of helping patients in the home to assist in their care by learning about their condition and how to effectively participate in managing their care and recovery.
Anyone (self, family, pastor, friend) may make a referral; however, a physician or certified nurse practitioner must order home health care services.
